Hum...are you sure you didn't miss a map? Fox, Bedwell, Mutiny & Myself did it several days ago and we all got the achievement.

Fred & I were playing T-Hunt last night and I made Officer.

Im positive. I can go to select mission, and each map in every scene has a check mark. For both 'normal' and 'realistic'.
